HIPAA notice of privacy practices

Your information. Your rights. Our responsibilities.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your rights

You have the right to inspect or obtain a paper or electronic copy of your medical record, ask us to correct your record, request confidential communications, ask us to limit certain uses or disclosures, receive an accounting of certain disclosures, obtain a paper copy of this notice, choose someone to act for you, and file a complaint if you believe your privacy rights were violated.

If you pay for a service in full out of pocket, you may ask us not to share information about that service with your health insurer for payment or health care operations. We will honor the request unless a law requires us to share the information.

Your choices

For certain health information, you may tell us your choices about what we share. This may include sharing information with family or friends involved in your care, disaster-relief situations, fundraising communications, and other uses where your authorization is required. We will not sell your health information or use it for marketing where authorization is required without your written permission. You may revoke an authorization in writing, except to the extent we have already relied on it.

How we may use and share health information

We may use and disclose health information for treatment, payment, and health care operations. We may also use or disclose information when permitted or required for public health and safety, health oversight, workers’ compensation, research allowed by law, organ and tissue donation, coroners or funeral directors, law enforcement, judicial or administrative proceedings, and other legal requirements. Additional protections may apply to psychotherapy notes, substance use disorder records, genetic information, reproductive health information, and other specially protected records.

Our responsibilities

We are required by law to maintain the privacy and security of protected health information, provide this notice, follow the duties and practices described here, and notify affected individuals following a breach of unsecured protected health information when required. We will not use or disclose information in ways not described here unless you authorize us in writing.

Changes to this notice

We may change this notice, and the revised notice may apply to information we already maintain. The current notice will be available upon request, at our clinics, and on this website.
