This public website and ordinary email are not intended for emergencies or for sending sensitive medical information. Call 911 for an emergency. For questions about your medical record or care, call the clinic at 661-446-4050.

This website does not currently include an online patient intake or appointment form. We may receive information you choose to provide when you call us, email us, or otherwise contact the clinic, such as your name, contact information, and the reason for your inquiry.

The website stores your English or Spanish preference in your browser so it can remember your choice.
